Library Appointment Cards

I finally got so frustrated with teachers missing their scheduled times (or worse, showing up when they weren't scheduled), that I designed an easy appointment card to give them when they come in to sign up for time.

Besides handing them the card, on Monday mornings, I also tape a copy to the mailbox of any teacher signed up for library time that week as a reminder.

The card consists of a simple table, with the periods listed vertically on the left and the days of the week horizontally across the top. I write the teacher's name on the slip, put an "X" in any period/day box they're signed up for and VOILA.
