Q. How do I change my withholding?
A. You may change your withholding by completing a new W-4 form. Please submit the completed W-4 to payroll.
Q. Where do I report name/address changes?
A. Please complete and submit an address change form to either Personnel or Payroll. A new social security card must be shown for name change.
Q. What happens when I sign up for direct deposit?
A. If the direct deposit authorization agreement is submitted to Payroll by the deadline of the 5th of each month, the direct deposit will take effect with the following month's paycheck (i.e., submitted by April 5th, will take effect with May pay date). If submitted after the 5th, please allow for two pay dates (i.e., submitted after April 5th, will take effect with June pay date).
Q. How does jury duty affect my paycheck?
A. Employees keep the check received from the court and submit an absent sheet indicating time missed from work for jury duty. A deduction of $10 per day will be taken on the paycheck following submittal of the absence sheet.
